I will abide by the camp rules. I acknowledge that injuries can occur when playing sport and I give TVCC permission to seek medical help as requried. I will pay for any costs associated with this care. I have included all medical details that may affect me during camp. If I am under 18 years old I have permission from my guardians to attend, and they agree with the above statement regarding injury. I agree to the above statement.
